WHAT IS THE PROCESS?
You can contact us directly.
A brief phone call will help us determine your eligibility.
- Your Case Manager can contact us on your
behalf and assist you in initiating a referral.
- After we review your referral, you will be
contacted by our Homescapes Project Coordinator. Our first
meeting, which may occur either in your home or the hospital,
will be used to determine eligibility for this service. The
Project Coordinator will explain what is needed from you
for this meeting.
- A Home Visit will be set up to include a licensed
architect, a functional representative from the Rehab facility,
potentially the builder, and the Homescapes Project Coordinator.
- The architect will draw a design for your
review and approval. Once approved, we will have the paperwork
signed and begin work.
- We work with the local county to obtain approval
for a building construction permit if needed.
- Our Project Coordinator will schedule a licensed
builder to begin construction, coordinate the sub-contractors
and inspect the work.
- Our goal is to complete this project in the
shortest amount of time while providing the best possible
architectural and construction services. Our goal is to provide
the greatest level of independence, safety and quality of
life based on your individual needs.
